Case Management: what to expect

We will offer a ‘Meet and Greet’ visit were we will discuss the client and their situation, we can then establish whether we can work together.
 
Once instructed, we undertake an Immediate Needs Assessment (INA) which is used to identify the client’s needs, make recommendations for intervention for the first 12 months, with associated costs, through agreed goals.
 
The assessment involves a discussion, observation and meeting with the client and family. Expert reports can also be used as additional information.
 
The goals are reviewed regualrly, in liaison with the client and family. The Case Management process and intervention is flexible, depending on need. Regular face to face meetings, telephone and email contact is used to ensure effective communication.

 

Case Management is defined by the Case Management Society UK (CMSUK)
as a collaborative process which assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual’s health, social care, educational and employment needs, using communication and available resources to promote quality, cost effective outcomes.

We offer support with…

Care: we can research, instruct and monitor care agencies to provide care packages for our clients.  We work closely with Human Resources (HR) and payroll providers to cost, advertise, interview and place support workers directly to work with clients. We then work with the family, clients and wider team to write a care plan, risk assessments etc. We arrange and review training needs and undertake supervision of support staff.

Therapy: once a need for therapy is established, we research and instruct appropriately skilled and experienced therapists. We make sure all therapists have the appropriate DBS checks and insurances and have the skills required. We ensure that the therapists are working towards their stated goals and actively encourage team working between all of the therapists and family. This could include Occupational Therapy, physiotherapy, Speech and Language Therapy, massage therapy, Clinical Psychology.

Medical care: we work closely with consultants, community nurses and those involved in the clients care to ensure that their medical needs are met in a timely manner.  We are able to refer to and liaise with private providers, if this is the most appropriate solution.

Education: we have experience in working within the EHCP (Education Health and Care Plan) process. We work with specialist education solicitors and consultants to ensure that we achieve a robust, accurate document and liaise with the school and Local Authorities to monitor the therapy, social and medical provision achieved. We regularly attend annual reviews and school meetings to support the working relationship between statutory services and private teams. We also work with a number of educational psychologists.

Accommodation: we liaise with solicitors, housing experts, property finders etc. to find appropriate rental or permanent properties which can be adapted to meet the client’s needs. We have the expertise to view potential properties, instruct architects/surveyors and equipment providers and work with the team to ensure that any adaptations and equipment is appropriate, correctly installed and maintained.

Equipment: we are able to assess and make recommendations for equipment, working closely with external providers, for example wheelchair companies, assistive technology providers, to cost, purchase, maintain and review equipment.

Transport: we work with a number of wheelchair accessible vehicle converters to assess for and recommend specific vehicles to meet the clients and families needs. This involves arranging and attending demonstrations of vehicles and securing the most appropriate funding.

Holidays: we can arrange holidays for clients, families and care teams, considering transport, accommodation, carer accommodation etc.

Finances: we can review the clients/families statutory benefits to ensure that they are receiving the correct benefits.
